A Case of Transfusion Associated Graft versus Host Disease after Open Heart Surgery

Abstract
Transfusion associated graft versus host disease(GVHD) is known to occur in patients whose immune systems are immature or impaired. It has also been recognized for several years that some patients who are not immunosuppressed and who die after
transfusion have the same clinical feature of transfusion associated GVHD; such as high fever, erythroderma, liver dysfuction, pancytopenia. We report a fatal case of transfusion associated GVHD in an immunocompetent female patient after being
transfused for open heart surgery. The patient was a 17 year-old female who underwent open heart surgery at Korea University Guro hospital in march, 1992. During operation, she received 3 units of fresh whole blood donated by her parents and
brother.
The operation was successful. On the 5th postoperative day, fever developed and graduailv-followed by wide spread erythroderma, liver dysfunction, pancytopenia, and bone marrow failure. She expired on the 30th postoperative day, with clinical
features
of sepsis. Skin biopsy showed findings of acute graft versus host disease, and bone marrow biopsy revealed aplastic marrow.
